As a member of the clinical faculty at the University of Rochester, he took active part in undergraduate education and postgraduate orthopaedic training. He developed highly successful and enviable practice in the area of his great interest and expertise - the joint replacement and arthroscopic surgery over a period of 15 years in Rochester. However, a strong desire to return to Pakistan and serve his own people motivated Prof. Umar to leave USA in 1990. He joined Aga Khan University and Hospital (Karachi) as a full time academic faculty member. At the Aga Khan University, he pioneered the development of arthroscopic surgery and joint replacement work from the scratch.

He was promoted to professor orthopaedic surgery in 1995 along with appointment of chief of orthopaedic services. During ten year service at Aga Khan University, Prof. Umar dedicated his life in training and developing orthopaedic faculty to create a department of international standard. He is proud to be a founding faculty member of orthopaedic residency training program at the Aga Khan University and its Director from 1995 – 2000.

During this period more than one dozen orthopaedic surgeons have been trained who are serving nationally and internationally. Prof. Umar has been very active in the organization and development of orthopaedic surgery in Pakistan. He is a founding member of Pakistan Society for surgery of the hand and has served as its general secretary and president from 1995 - 1998.

He also had the honour to serve as secretary general and president of Pakistan Orthopaedic Association from 1996 - 1999. Prof. Umar changed his status to a part-time professor in the department of surgery at AKU in June 2000 to devote a significant amount of his time to the Institute of Orthopaedic and Surgery at Karachi, as its surgeon-in-chief. In December 2008 Prof. Umar has retired from AKU and dedicated himself for active role to develop department of orthopaedic surgery at South City Hospital. Prof. Umar laid the foundation as chief of orthopaedic surgery at South City Hospital in April 2008. A group of highly qualified orthopaedic surgeons was organized under the name of orthopaedic associates. At present which has four consultants who specialize in trauma, paediatric, orthopaedic oncology n adult reconstructive surgery.

Institute of Orthopaedics & Surgery Hospital has state-of-the-art facilities and is highly suited for modern and advanced orthopaedic surgery i.e. primary joint replacement, revision surgery, reconstructive knee surgery, limb sparing surgery with custom made implants and trauma reconstruction.
